1. RATES & BILLING

4. HEALTH & VACCINATION REQUIREMENTS

The Dog must be current on [rabies, DHPP, Bordetella / your required list] and on
flea and tick prevention. The Owner will disclose any medications, medical
conditions, or special needs in advance. The Sitter may decline or send home a
dog that arrives visibly ill, with fleas, or otherwise unfit to board safely
around other dogs.


5. MULTI-DOG HOUSEHOLD

Dogs from the same household board together unless the Owner requests otherwise.
Each dog is listed above and covered by this contract. A solo-only stay (no other
guests) is available at $[______] and is subject to availability.

7. LIABILITY & WAIVER

Boarding, walking, and play carry inherent risks. This contract covers the
business terms of the stay. The separate signed Care & Liability Waiver and
Emergency Veterinary Authorization cover consent, the vet spend cap, and the
liability release, and must be signed before drop-off.

are authorized to sign on the Owner's behalf.
